Regional ultrasonographic screening for developmental dysplasia of hips in infants and analysis of ultrasonic bone density parameter characteristics

Regional ultrasonographic screening for developmental dysplasia of hips in infants and analysis of ultrasonic bone density parameter characteristics

Abstract

Objective To evaluate the screening value of Graf method for high-frequency ultrasound in assessing developmental dysplasia of hips(DDH) in infants in Fengtai District of Beijing, and to analyze the risk factors for DDH occurrence in the region and the correlation between ultrasonic bone density parameters and DDH. Methods A total of 383 infants(766 hip joints) who underwent hip ultrasound examination at Beijing Fengtai Teaching Hospital from November 2023 to December 2024 were selected. Graf method was used for examination, and α angle and β angle were measured. The standard classification (type I: mature hip joint, type IIa and above: abnormal hip joint) was adopted. Bone density parameters and relevant clinical data were collected, and multivariate logistic regression analysis was employed to identify the risk factors for DDH. Results Ultrasound detected developmental dysplasia of the hip in 52 infants, involving a total of 61 abnormal hips. The overall detection rate was 7.96%(61/766), with type IIa being the predominant subtype 96.72%(59/61). Breech presentation, oligohydramnios, primiparity, and high birth weight were independent risk factors (all P<0.05). No statistically significant association were found between gender and ultrasonic bone density parameters. Conclusions The Graf method for early ultrasound screening of infants can timely detect DDH. Breech presentation, oligohydramnios, first birth, and high birth weight are risk factors for DDH occurrence, whereas ultrasonic bone density parameters may not be major influencing factors for DDH in this region.
