摘要

目的 探究新兵入伍集训期间相关生理指标的变化及其意义。方法 选取某部新兵126人,按照训练大纲集中2个月体能训练,进行跟踪式定期检查。以入伍集训开始、3 d、1周、2周、1个月、2个月为6个检测时间点,清晨起床后测脉搏、血压、身高、体重,空腹抽取静脉血,行血常规、C-反应蛋白(C-reactive protein,CRP)、心肌酶、肌红蛋白、肌钙蛋白、血清肌酸激酶同工酶、肝功能、肾功能、B型钠尿肽(brain natriuretic peptide,BNP)、超声心动图检查。1 km跑步后立即测心率及脉搏。结果 新兵集训后基础生理指标无明显变化,最高心率升高、平均心率下降、1 km跑成绩提升;集训后新兵血清中心肌肌钙蛋白Ⅰ(cardiac troponin,cTnⅠ)、血清肌酸激酶同工酶(MB isoenzyme of creatine kinase,CK-MB)数值在生理值范围内升高,谷草转氨酶(aspartate aminotransferase,AST)、血清肌酸激酶(creatine kinase,CK)、乳酸脱氢酶(lactate dehydrogenase,LDH)、肌红蛋白(myoglobin,MYO)明显升高并远高于生理值,BNP轻度升高后恢复生理水平,CRP无变化;超声检测见EF值无明显改变,左室每搏输出量明显提升,室间隔厚度虽轻度下降但仍处于正常生理值范围内。结论 新兵集训后心肺功能明显提升,集训质量达标;集训可导致部分心肌酶增高,但并未出现器质性心肌损害。

Abstract

Objective To explore the changes and significance of physiological indexes of recruits in intensive training. Method One hundred and twenty-six recruits in mobile forces were selected as research targets. According to the training program, they concentrated on physical training for 2 months and carried out follow-up regular inspection. Six test time points are the beginning of the training, 3 days of training, 1 week, 2 weeks, 1 month and 2 months of training; After getting up, pulse, blood pressure, height and weight were measured; Fasting venous blood was drawn for blood routine to examine C-reactive protein (CRP), myocardial zymogram, myocardial infarction, liver function, renal function, Brain Natriuretic Peptide (BNP), echocardiography. Blood oxygen and pulse were immediately measured after 1km running. Results There was no significant change in the basic physiological indicators of the recruits after the army recruits training. The maximum heart rate increased, the mean heart rate decreased, and the performance of one-kilometer running improved. After the training, the values of cardiac troponin(cTnT) and MB isoenzyme of creatine kinase(CK-MB) in blood serum increased within physiological range, Aspartate aminotransferase(AST), creatine kinase(CK), Lactate dehydrogenase(LDH) and Myoglobin(MYO) significantly increased and were much higher than the physiological values, and the physiological level of BNP recovered after mild elevation, but there was no change in CRP. Ultrasonic cardiogram showed that there was no significant change in EF value. The left ventricular stroke output significantly increased, and the interventricular septal thickness slightly decreased but remained at a range of physiological condition. The cardiopulmonary function was significantly improved after recruit training and still met the standards. Conclusion Utrasonic cardiogram and myocardial enzymology detection system may assess the myocardial injury and timely screen out high-risk cases prone to cardiovascular events in the recruit training.
