摘要

目的 比较发散式体外冲击波疗法(RESWT)与神经肌肉电刺激(NMES)对产后腹直肌分离(DRA)的疗效。方法 本研究为随机对照试验,选取2024年1-12月于解放军总医院第七医学中心康复医学科就诊的产后DRA患者72例,随机分为观察组和对照组,每组36例。对照组接受NMES治疗,1次/隔日,3次/周;观察组采用RESWT进行治疗,1次/周;两组均在康复师指导下进行核心稳定性训练,1次/d,每周训练5 d。治疗5周后,比较治疗前后两组腹围、腹直肌间距(IRD),评估腰背痛评分和生活质量评分(WHOQOL-BREF)。结果 72例在研究过程中无脱落,全部进入结果分析。治疗5周后,两组腹围、IRD均缩小,腰背部疼痛(VAS)评分均降低,WHOQOL-BREF升高,差异均有统计学意义(P<0.05);观察组与对照组比较,腹围和IRD差异无统计学意义(P>0.05),腰背部疼痛症状和生活质量改善差异有统计学意义(P<0.05)。结论 RESWT与NMES均能有效促进产后DRA恢复,减轻腰背疼痛,提高生活质量,RESWT在疼痛管理方面效果更优。

Abstract

Objective To compare the efficacy of radial extracorporeal shock wave therapy(RESWT) and neuromuscular electrical stimulation(NMES) in the treatment of postpartum diastasis recti abdominis(DRA). Methods This study was a randomized, double-blind trial. A total of 72 patients with postpartum DRA who visited the Rehabilitation Medicine Department of the 7th Medical Center of PLA General Hospital from January 2024 to December 2024 were selected and randomly divided into an observation group(n=36) and a control group(n=36). In the control group, NMES was performed, once every other day, 3 times a week, while RESWT was applied in the observation group, once a week. Both groups received core stability training under the guidance of a rehabilitation therapist, once a day, 5 days a week. After 5 weeks of treatment, the abdominal circumference and inter recti distance(IRD) were measured, the visual analogue scale(VAS) and life quality score(World Health Organization Quality of Life-BREF,WHOQOL-BREF) were compared before and after treatment. Results There was no dropout during the study, and all patients entered the result analysis. After 5 weeks of treatment, the abdominal circumference and IRD were reduced, the VAS of lumbar decreased, the life quality score increased in both groups, and the differences were statistically significant (P<0.05). Compared with the control group, the abdominal circumference and IRD in observation group showed no significant difference (P>0.05),but there was statistically significant difference in the improvement of low back pain symptoms and quality of life (P<0.05). Conclusions Both RESWT and NMES can effectively promote the recovery of postpartum DRA, reduce low back pain, and improve life quality. The former is more effective in pain management.
