摘要

目的系统总结子宫阔韧带疝的临床表现、CT影像学特征、治疗方式及解剖学特点。方法系统检索中英文数据库,纳入手术确诊的子宫阔韧带疝个案报告,提取人口学、既往史、临床表现、CT征象、手术发现及治疗等数据,并进行统计学描述分析。结果共纳入77例患者,平均年龄(43.11±12.10)岁。基于有效样本统计,82.70%有妊娠史,53.80%有腹部或盆腔手术史,98.68%表现为腹痛,77.63%伴有呕吐,CT诊断率为58.33%,常见CT征象是子宫附件周围梗阻点(75%)和子宫直肠移位(25%);56.1%是腹腔镜手术,且2010年后应用显著增加(P<0.001);左侧缺损(54.93%)和左侧疝(59.15%)更常见,回肠是最常疝入器官(43.42%),Hunt孔窗型和Cilley 1型最常见。结论子宫阔韧带疝好发于有妊娠史及腹部手术史中年女性,临床表现不典型,CT是术前诊断关键,腹腔镜手术已成为主流手术方式。

Abstract

Objective To systematically summarize the clinical manifestations, CT imaging features, treatment approaches, and anatomical characteristics of uterine broad ligament hernia (BLH). Methods A systematic search was conducted in Chinese and English databases to identify case reports of surgically confirmed uterine BLH. Data on demographics, medical history, clinical manifestations, CT findings, surgical discoveries and treatment were extracted and statistically analyzed. Results A total of 77 patients were included, with an average age of (43.11±12.1) years. Analysis of valid samples revealed that 82.70% had a history of pregnancy, and 53.80% had a history of abdominal or pelvic surgery. Clinical manifestations included abdominal pain in 98.68% of patients and vomiting in 77.63%. The preoperative diagnostic rate by CT was 58.33%. Common CT signs were adnexal obstructive points (75%) and uterine rectal displacement (25%). Laparoscopic surgery was performed in 56.1% of the cases, and the application significantly increased after 2010 (P<0.001). Left-sided defects (54.93%) and left-sided hernias (59.15%) were more frequent, the ileum was the most common herniated organ (43.42%), and the Hunt fenestrated type and Cilley type 1 were the most prevalent classifications. Conclusions Uterine BLH predominantly affects middle-aged women with a history of pregnancy or abdominal surgery. Its clinical manifestations are atypical. CT is crucial to preoperative diagnosis, and laparoscopic surgery has become the mainstream surgical method.
