目的 探讨高原环境下胆囊结石伴慢性胆囊炎患者行经脐单孔腹腔镜胆囊切除术(TUSLC)的安全性和有效性;方法 选取2024-01至2025-12武警西藏总队医院收治的73例胆囊结石伴慢性胆囊炎患者为研究对象,随机分为对照组(传统腹腔镜胆囊切除术,CLC,n=38)和试验组(经脐单孔腹腔镜胆囊切除术,TUSLC,n=35),比较两组患者手术时间、出血量、切口满意度、住院时间和术后疼痛评分。结果 试验组患者在出血量和住院时间方面与CLC组无统计学差异(P>0.05)。试验组在术后疼痛、切口满意度方面显著优于对照组(P<0.05),但试验组手术时间长于对照组。结论 在高原地区胆囊结石伴慢性胆囊炎患者的治疗中,TUSLC的安全性和有效性与CLC相当,TUSLC患者术后切口疼痛较轻,切口外观满意度较高,总体提升了患者术后生活质量。
Abstract
Objective To investigate the safety and efficacy of transumbilical single-port laparoscopic cholecystectomy (TUSLC) in patients with gallbladder stones and chronic cholecystitis in a high-altitude environment. Methods A total of 73 patients with gallbladder stones and chronic cholecystitis who were admitted to Tibet Autonomous Regional Corps Hospital of Chinese People’s Armed Police Force from January 2024 to December 2025 were selected as the research subjects and randomly divided into a control group by traditional laparoscopic cholecystectomy (CLC, n=38) and an experimental group by TUSLC (TUSLC, n=35). The operation time, blood loss, satisfaction with the incision, hospital stay, and postoperative pain score were compared between the two groups. Results There was no statistically significant difference in blood loss or hospital stay between the two groups (P>0.05). The experimental group showed significantly better postoperative pain and incision satisfaction compared with the control group (P<0.05),but the operation time of the experimental group was longer than that of the control group. Conclusions In the treatment of patients with gallbladder stones and chronic cholecystitis in high-altitude region, the safety and efficacy of TUSLC are comparable to those of CLC. The patients in the TUSLC group experienced less postoperative incision pain and higher satisfaction with the incision appearance, which overall improves the postoperative quality of life of patients.
