摘要

目的 比较弹性髓内针(ESIN)分别联合类固醇激素与自体骨髓(ABM)注射治疗儿童长骨单纯性骨囊肿(UBC)的疗效与安全性。方法 回顾性分析2015-01至2023-12解放军总医院第一医学中心和第七医学中心收治的UBC患儿45例,分为A组(21例,ESIN+类固醇激素)和B组(24例,ESIN+ABM)。术后定期行影像学随访,依据Capanna标准评估囊肿愈合情况。结果 45例均获随访,平均随访40.4个月。A组总有效率为90.5%(19/21),B组总有效率为91.7%(22/24),两组差异无统计学意义(P=0.89)。45例中,治愈28例(62.2%),部分愈合13例(28.9%),复发2例(4.4%),治疗无效2例(4.4%)。结论 ESIN分别联合类固醇激素与ABM注射均是治疗UBC安全有效的微创方法。

Abstract

Objective To compare the efficacy and safety of elastic stable intramedullary nailing (ESIN) combined with corticosteroid versus combined with autologous bone marrow (ABM) injection in the treatment of unicameral bone cyst (UBC) in the long bones of children. Methods A retrospective analysis was performed on 45 children with UBC admitted to the First and the Seventh Medical Centers of the PLA General Hospital from January 2015 to December 2023. The patients were divided into group A (n=21, ESIN plus corticosteroid) and group B (n=24, ESIN plus ABM injection). Postoperative imaging follow-up was performed regularly, and cyst healing was evaluated according to the Capanna criteria. Results All the 45 patients completed follow-up, with a mean follow-up duration of 40.4 months. The overall effective rate was 90.5% (19/21) in group A and 91.7% (22/24) in group B, with no statistically significant difference between the two groups (P=0.89). Among the 45 cases, 28 (62.2%) achieved complete healing, 13(28.9%) had partial healing, 2(4.4%) developed recurrence, and 2(4.4%) showed no response to treatment. Conclusions Both ESIN combined with corticosteroid and ESIN combined with ABM injection are safe and effective minimally invasive methods for the treatment of UBC.
