中枢神经系统单核细胞增生李斯特菌感染1例

摘要

报道1例中枢神经系统单核细胞增生李斯特菌感染患者。患者为60岁男性,急性起病,临床表现为高热、头痛、寒战及脑膜刺激征阳性,发病前有不洁饮食史。实验室检查示外周血中性粒细胞显著升高,脑脊液呈浑浊状,白细胞计数升高,葡萄糖及氯化物降低。头颅MRI显示脑桥左侧及双侧脑室周围对称性T₂高信号伴强化。脑脊液细菌培养及质谱鉴定确诊为单核细胞增生李斯特菌感染。经青霉素钠、庆大霉素及复方磺胺甲噁唑三联治疗后临床症状逐步缓解。随访3个月,仍遗留认知障碍及脑室扩大。
