目的探讨妇科腹腔镜手术中并发泌尿系统损伤的原因及防治。方法回顾性分析我院2005-01至2012-12间妇科腹腔镜手术并发泌尿系损伤的临床资料。结果1120例腹腔镜手术发生泌尿器官损伤6例(0.53%),其中膀胱损伤4例,输尿管损伤1例,输尿管合并膀胱损伤1例。4例膀胱损伤、1例输尿管损伤术中诊断并处理,1例输尿管合并膀胱损伤术后诊断行二次手术修补,预后良好。结论妇科腹腔镜手术并发泌尿系损伤是比较严重的手术并发症,正确掌握手术指征、提高手术技巧、适时中转开腹,可减少此类损伤,早期正确处理可改善预后
Abstract
Objective To investigate the main reason of urinary tract injury in gynecological laparoscopy and to discuss its treatment and prevention. Methods The clinical data of patients in our department who suffered urinary tract injury in laparoscopic operation during Januarary 2005 and December 2012 were reviewed retrospectively. Results There were 6(0.53%) urinary tract injuries among 1120 gynecologic laparoscopies.4 bladder injuries and 1 ureteral injury were treated during the surgery, The other one was cured after one more operation. Conclusion Urinary tract injury is a serious complication of gynecological laparoscopy and should be pay more attention to.The advanced procedures,the correct surgery indications and timely conversion to laparotomy are keys to reduce these injuries,and the right early treatment can improve the prognosis.
