目的调查扬州地区患者人群中红细胞血型不规则抗体的检出率及不规则抗体特异性的分布特点。方法对2010-01至2012-12间18625例扬州地区住院患者进行抗体筛查，应用盐水法、抗球蛋白法、聚凝胺法检测不规则抗体，并对抗筛结果阳性者鉴定其抗体特异性。结果共鉴定确认血型不规则抗体136例，占总筛查人群的0.73%，其中男21例，女115例;不规则抗体分别为自身抗体45例，同种特异性抗体77例。结论不规则抗体产生的主要患者人群为有输血史、妊娠史人群，自身抗体主要发生在中老年及自身免疫性疾病患者人群。输血前检测不规则抗体必不可少，特别对有输血史或妊娠史的患者更为重要，对保证患者输血安全、减少溶血性输血反应具有重要意义。

Objective To investigate the distribution of irregular red blood cell antibodies, the detection rate and the irregular antibody specificity among patients in Yangzhou area. Methods From January 2010 to December 2012, 18 625 patients in Armed Police Forces Jiangsu Corps Hospital or clinic patients were examined by antibody screening method, application of saline, antiglobulin method, polybrene method for detection of irregular antibodies, and the positive screening results against the identification of antibody specificity. Results Irregular antibody were identified in 136 cases, accounting for 0.73% of the total screened population, of whom 21 were male, 115 female. Irregular antibodies were autoantibodies in 45 cases and homospecific antibodies in 77 cases. Conclusion Patients with irregular antibodies have blood transfusion history and, for pregnant women, autoantibodies mainly occur in the elderly and in patients with autoimmune disease.
