摘要

目的了解耳鸣患者中枢代谢特点,探讨掩蔽治疗时中枢代谢变化规律。方法选取5例正常听力的耳鸣患者,同时选取20例行正电子发射计算机断层扫描/计算机体层扫描(positronemissiontomography/computedtomography,PET/CT)检查的健康人作对照组,对耳鸣患者及健康人的左右侧颞横回区及海马标准化摄取值进行比较,同时对5例耳鸣患者每侧颞横回及海马区掩蔽治疗前及掩蔽治疗时的标准化摄取值进行比较。结果5例耳鸣患者中有4例的双侧颞横回及海马区代谢不对称,均表现为左侧明显高于右侧,另1例耳鸣患者双侧代谢对称;而对照组上述区域代谢基本对称,两组间的不对称指数具有统计学差异(P<0.01)。5例耳鸣患者掩蔽治疗时,可见患侧双侧听皮质标准化摄取值均较未给声时升高,每侧掩蔽治疗前、后均有统计学差异(P<0.01)。结论耳鸣可能与左侧颞横回区及海马的高代谢有关,掩蔽治疗可能是在中枢层面通过影响相关听功能区域的代谢水平而产生作用。

Abstract

Objective To study the central metabolic characteristics and the change in central metabolism in patients with tinnitus undergoing masking therapy. Methods Select Five subjects with normal hearing tinnitus and, at the same time, twenty normal people who have undergone PET-CT examination were recruited. The tinnitus patients’ standardized uptake values(SUV) of right and left transverse temporal gyri and hippocampal gyrus were compared with those in normal subjects. SUV of 5 tinnitus patients were compared on each side of transverse temporal gyri and hippocampal gyrus before masking therapy with those having masking therapy. Results Of 5 cases of tinnitus, transverse temporal gyri and hippocampal gyrus metabolism was asymmetric in 4 cases, and the left metabolism was significantly higher than the right side on all performance. The other one was normal. All metabolism in normal control group was symmetrical. Between the two groups, the asymmetric indexes were significantly different (P<0.01). And in 5 cases of tinnitus with masking therapy, SUV of auditory cortex with sound in one side masking therapy was higher than that without sound, each side before masking therapy and after had significant difference (P< 0.01). Conclusions The high metabolism of the left transverse temporal gyri and hippocampal gyrus is associated with tinnitus. Masking therapy has curative efficacy and may affect the metabolic level of auditory cortex functional area.
