目的研究老年人血脂代谢水平与结直肠腺瘤发病的关系。方法 选取行结肠镜检查的60岁以上老年患者265例, 根据是否患有结直肠腺瘤分为腺瘤组及对照组, 测定两组血总胆固醇、三酰甘油、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)水平, 并进行对比分析。结果 腺瘤组血总胆固醇、三酰甘油水平较对照组升高(P＜0.05), HDL-C水平下降(P＜0.01);腺瘤组中HDL-C降低的比例多于对照组(P＜0.05);绒毛状腺瘤组血清总胆固醇值(4.96±0.85) mmol/L较管状腺瘤组(3.98±0.23)mmol/L及混合腺瘤组(4.57±0.73)mmol/L明显升高(P=0.018)。结论 高胆固醇血症、高三酰甘油血症与结直肠腺瘤的发生、发展及恶变有关, HDL-C可能对其有抑制作用。

Objective To study the relationship between serum lipid level and colorectal adenoma in elderly pations. Methods The clinical data for 265 pations aged 60 and over of cadre wards from August 2010-August 2013 in this hospital underwent colonoscopy were retrospectively analyzed. They were divided into two groups: patients with colorectal adenoma and those without colorectal adenoma. The serum total cholesterol, triglyceride, HDL cholesterol and LDL cholesterol levels in the two groups were analyzed. Results The levels of total cholesterol, triglyceride in serum of patients with colorectal adenoma were much higher than those in the control group (P<0.05), the levels of HDL cholesterol were much lower (P<0.01). HDL cholesterol adenoma group decreased significantly than that in control group (P<0.05). Conclusions The findings of this study suggest that dyslipidemia may affect the incidence of colorectal adenoma.Total cholesterol, triglyceride in serum can promote occurrence and deterioration of colorectal adenomatous, but HDL cholesterol in serum has inhibiting effect.
