摘要
目的 探讨笑气(nitrous oxide, N2O)吸入清醒镇痛在结肠镜检查中的有效性与安全性,进一步评价其临床应用价值。方法 将532例进行结肠镜检查的患者随机分为N2O组255例和纯氧组277例,观察比较两组患者术前、术中、术后心率(heart rate,HR)及血氧饱和度(blood oxygen saturation,SPO2)波动情况;术后患者和内镜操作医师分别填写有效性调查问卷:包括内镜医师对操作过程的评价,分为顺利、一般、不顺利;患者操作过程中的不适,分为轻度、中度、重度;患者是否愿意再次进行同样的操作,分为是、否,并在视觉模拟疼痛评分(visual analogue pain score criteria,VAS)量表上标记,对所得数据进行统计学分析。结果 N2O组与纯氧组患者术前、术中、术后HR及SPO2变化,差异无统计学意义; N2O组医师评价操作过程的平稳性、患者的耐受性明显优于纯氧组,差异有统计学意义(P＜0.01);N2O组患者感到操作过程轻微不适的比例(81.18%)及愿意再次接受检查的比例(67.45%)明显高于纯氧组(53.79%,42.60%),差异有统计学意义(P＜0.01)。结论 N2O吸入清醒镇痛可提高结肠镜检查的舒适度,操作简便、安全,值得临床推广应用。
Abstract
Objective To investigate the effectiveness and safety of nitrous oxide for colonoscopy and evaluate its clinical application value. Methods A total of 532 patients were randomly divided into N2O group (inhalation with nitrous oxide ) and O2 group (inhalation with pure oxygen).The heart rate(HR) and blood oxygen saturation (SPO2) were observed before, during, and after the colonoscopy. The reaction and feeling of patients and doctors were evaluated by visual analogue scale(VAS). Results There was not significant difference in HR and SPO2 between the N2O group and O2 group. The satisfaction rate of both doctors and patients in the N2O group was higher than O2 group,withsignificant difference(P＜0.01). Conclusions Nitrous oxide for colonoscopy is comfortable and safe, and is worthy of widespread use.
