目的 比较诺舒(NovaSure)和左炔诺孕酮缓释系统(levonorgestrel-releasing intrauterine system，LNG-IUS)治疗异常子宫出血(abnormal uterine bleeding，AUB)的疗效。方法 回顾性分析2013-12至2014-08就诊天坛医院的AUB患者，并且行NovaSure子宫内膜去除术(NovaSure组)和LNG-IUS宫内放置术(LNG-IUS组)的病例资料。其中，NovaSure组36例; LNG-IUS组42例。比较两组的一般资料及术后1年的随访资料。结果 NovaSure组及 LNG-IUS组术后1年ΔPBAC评分分别为(324.1±139.7)分、(255.8±96.1)分，两组比较差异有统计学意义(P<0.05);Δ痛经评分分别为(2.94±1.55)分、(3.73±1.95)分，两组比较差异有统计学意义(P<0.05);1年闭经率分别为66.6%、31%，痛经缓解率分别为59.1%、89.2%，差异均有统计学意义(P<0.05)。结论 NovaSure和LNG-IUS治疗AUB均疗效显著，但NovaSure治疗月经过多更显效，闭经率更高;而LNG-IUS缓解痛经更显著，痛经缓解率较高。
Abstract
Objective To investigate the clinical effectiveness of treating the abnormal uterine bleeding by using NovaSure and levonorgestrel-releasing intrauterine system.Methods AUB patients who had been admitted to Tiantan Hospital for NovaSure endometrial ablation and levonorgestrel-releasing intrauterine system between December, 2013 and August, 2014 were retrospectively analyzed. There were 36 patients for NovaSure and 42 patients for levonorgestrel-releasing intrauterine system to compare the information about present and one-year after the operation.Results The scores of NovaSure and LNG-IUS one year after the operation were (324.1±139.7)points and (255.8±96.1)points, respectively (P<0.05).The scores of menalgia were (2.94±1.55) points and (3.73±1.95)points, respectively(P<0.05). The one-year amenorrhea rates were 66.6% and 31.0%, respectively (P<0.05). The rates of relieves of menalgia were 59.1% and 89.2%, respectively (P<0.05).Conclusions NovaSure and LNG-IUS to treat AUB are both significantly effective. It is more effective to treat hypermenorrhea and lead to amenorrhea by NovaSure. It is better to relieve menalgia and the rates of that are higher by LNG-IUS.
