目的 探讨亲水性纤维含银敷料对老年糖尿病患者手术切口感染的预防效果。方法 选取2015-08至2016-12在医院进行下肢骨科手术治疗的老年糖尿病患者100例(年龄>60岁),随机分为对照组和观察组,每组50例,对照组常规采用普通纱布敷料,观察组采用亲水性纤维含银敷料,比较两组患者切口感染率、切口愈合情况、术后血常规白细胞水平及患者满意度。结果 观察组患者切口感染发生率(2.0%)明显低于对照组(16.0%),差异有统计学意义(P<0.05);观察组患者切口甲级愈合率(84.0%)高于对照组(60.0%),并且乙级愈合率(16.0%)低于对照组(34.0%),差异有统计学意义(P<0.01);观察组无丙级愈合,对照组有3例,并且观察组术后3 d白细胞水平明显低于对照组,差异有统计学意义(P<0.01)。结论 亲水性纤维含银敷料可有效预防老年糖尿病患者切口感染的发生,且有效促进切口愈合,提高患者满意度,具有很大的临床应用价值。
Abstract
Objective To explore the effect of hydrophilic fiber containing silver dressings on prevention of incision infection among elderly lower limb surgical patients with diabetes.Methods One hundred elder patients (age >60) with diabetes were equally divided into the control group and observation group. In addition to routine care, the control group was treated with common gauze dressings while the observation group used hydrophilic fiber containing silver dressings. The infection rate, healing of incision,postoperative levels of routine blood leucocytes and levels of satisfaction of patients were compared between the two groups.Results The incision infection rate of the observation group was lower than in the control group (2.0% vs 16.0%),P<0.05. The Class A healing rate of the observation group was higher than in the control group (84.0% vs 60.0%, P<0.01) but the Class B healing rate was lower (16.0% vs. 34.0%, P<0.05).There was no patient with Class C healing in the observation group, but there were three cases in the control group. Three days after surgery, the white blood cell levels in the observation group was obviously lower than in the control group (P<0.01).Conclusions Hydrophilic fiber containing silver dressings can effectively prevent the occurrence of infections of incisions among elderly patients with diabetes, promote the healing of incisions, and improve patients’ level of satisfaction, so this approach is of great clinical value.
