目的 探讨麻醉方法对烟雾病患者脑血管搭桥手术术后早期认知功能的影响。方法 选择2015-05至2016-03在天坛医院治疗的74例烟雾病患者,分为全静脉麻醉组(total intravenous anesthesia, T组)和吸入麻醉组(inhalation anesthesia, I组),全部患者择期行血管搭桥手术,术中实施全身麻醉,所有患者在术前及术后7 d分别接受简易精神状况量表(MMSE)和蒙特利尔认知功能量表(MoCA)检测,评估术后早期认知功能。结果 与术前相比,两组患者术后7 d MMSE评分各项均无统计学差异。两组患者MoCA评分中记忆力一项有明显改善,差异有统计学意义(P<0.05),其余各项均无统计学差异。两组患者之间评分比较无统计学差异。结论 脑血管搭桥手术对患者早期记忆功能有明显改善;全静脉麻醉和吸入麻醉对脑血管搭桥手术术后早期认知功能的影响无明显差异。
Abstract
Objective To explore the effect of cerebral vascular bypass surgery on early postoperative cognitive functions in patients with moyamoya disease under intravenous anesthesia and inhalation anesthesia. Methods A total of 74 patients with moyamoya disease were divided into the total intravenous anesthesia group (T Group) and inhalation anesthesia group (I group). All the patients received cerebral vascular bypass surgery. The cognitive functions of patients were assessed before surgery and 7 days after surgery by mini mental state examination (MMSE) and Montreal Cognitive Assessment (MoCA). General anesthesia was performed during the operations. Results After operation, there was no significant difference in MMSE scores of patients’ cognitive functions. As for MoCA scores, only memory function improved obviously in both groups, and the difference was statistically significant (P<0.05). There was no significant difference between the two groups in other aspects. Conclusions Vascular bypass surgery can improve the early memory function in patients with moyamoya disease, but has no significant influence on other cognitive functions. Neither total intravenous anesthesia nor inhalation anesthesia makes much difference to the early cognitive function of patients after cerebral vascular bypass surgery.
