低温等离子射频消融治疗下颈椎椎间盘源性眩晕的疗效

摘要

目的 探讨低温等离子射频消融术治疗下颈椎椎间盘源性眩晕的临床疗效。方法 选取2017-03至2018-06解放军总医院第三医学中心脊柱外科收治的78例颈椎间盘源性眩晕患者,于C形臂X线机引导下行低温等离子射频消融治疗,观察患者术后头晕改善情况。采用0-100视觉模拟评分法和眩晕障碍量表评估眩晕程度变化,采用改良MacNab行手术疗效的优良率评估。结果 术后各时间段眩晕VAS评分和DHI 评分较术前明显改善,差异有统计学意义(P<0.01);术后3 d及3、6、12个月的优良率分别为70.5%、77.0%、78.0%和82.0%。结论 低温等离子射频消融术是治疗下颈椎椎间盘源性眩晕的一种有效且安全方法。

Abstract

Objective To evaluate the efficacy of nucleoplasty coblation in patients with lower cervical discogenic vertigo.Methods Seventy-eight eligible patients were treated with nucleoplasty coblation guided by the G mode X-ray machine.The visual analogue scale (VAS) and Dizziness Handicap Inventory(DHI)were recorded before and after treatment.The improved Mac Nab criterion was adopted to evaluate the clinical efficacy.Results The VAS scores at each post-treatment time point were remarkably lower than before treatment,and the difference was statistically significant (P<0.01).The excellent and good rate of therapy at 3 days,3 months,6 months and 12 months after treatment was respectively 70.5%,77.0%,78.0% and 82.0%.Conclusions This study indicates that nucleoplasty coblation is an effective and safe approach to lower cervical discogenic vertigo.
