摘要

目的 探讨药源性肾损害(drug induced kidney injury,DIKI)的发生特点及相关因素,为临床用药提供参考。方法 收集联勤保障部队北戴河康复疗养中心2006—2020年发生DIKI报告250例,从患者的性别、年龄、发生时间、给药途径、ADR转归及关联性评价、涉及药物类别等方面进行统计分析。结果 250例DIKI中,女153例(61.20%),男97例(38.80%),多发生在服用药物后1~5年(27.60%);发生DIKI的年龄大于40岁的中老年患者210例(84.00%);有后遗症162例(64.80%);涉及的品种17类、69 种,其中以中药制剂居多(62.40%)。结论 加强中药制剂、解热镇痛抗炎药、抗菌药物的用药监测,减少药物对患者肾功能损害。

Abstract

Objective To investigate the regularities and related factors of DIKI, and provide reference for clinical drug use. Methods Two hundred and fifty cases of DIKI reported in our hospital between 2006 and 2020 were collected and analyzed statistically in terms of gender, age, time of occurrence, administration routes, outcomes, correlations with ADRs, and the types of drugs involved. Results Among these cases of DIKI, 153 were females (61.20%) and 97 were males (38.80%). Most of the cases of DIKI occurred 1 to 5 years after medication (27.60%). 210 of these cases were above 40 (84.00%), and 162 had sequelae (64.80%). Seventeen categories of drugs and 69 types of drugs were involved, the majority of which were traditional Chinese medicine preparations (62.40%). Conclusions Traditional Chinese medicine preparations, antipyretics, analgesics, anti-inflammatory drugs and antibiotics should be monitored more rigorously to reduce the damage of drugs to patients' renal function.
