目的 探讨浆细胞性乳腺炎不同治疗方法及其效果。方法 回顾性分析1997-01至2019-12在解放军总医院第三医学中心就诊的700例浆细胞性乳腺炎患者临床资料。治疗方式包括直接手术、抗结核治疗后手术、病因治疗后手术。病因治疗主要针对乳头溢液和乳头凹陷症状查找原因并积极治疗后手术。结果 直接手术者152例,18例(11.84%)复发。第二组为抗结核治疗组58例,疗效不佳,其中55例(94.83%)最后全部选择手术治疗。第三组病因治疗490例,其中溢液382例(77.96%),根据溢液分析并针对性治疗后手术者约115例,无一例再次发作;乳头内陷的108例中,有102例(98.15%)手术并同时纠正乳头内陷后无复发,另6例(5.56%)同时合并有乳头溢液,治疗溢液后症状减轻予手术治疗。结论 浆细胞性乳腺炎患者直接手术复发率高,抗结核抗炎治疗无效,控制乳头溢液再手术、乳头凹陷纠正后复发率低,疗效更确切。
Abstract
Objective To explore the different treatment methods and effects of plasma cell mastitis.Methods Retrospective analysis of 700 patients with plasma cell mastitis took three different treatment methods, namely, direct surgery, anti-tuberculosis treatment surgery, post-treatment surgery. The cause treatment mainly aims at finding the cause of nipple overflow and nipple depression symptoms and actively treating post-treatment surgery. Finally, the therapeutic effect of these three methods is statistically compared.Results In 152 cases of direct surgery, about 11.84% relapsed. The second group was 58 cases of anti-tuberculosis treatment, with poor efficacy, of which 55 cases (94.83%)were all surgically selected. The third group of cause treatment, there are 382 cases(77.96%)of overflow patients, according to the spill analysis and targeted treatment after surgery about 115 cases, none of the re-onset. In the third group, there were 108 patients with nipple insulation, of which 102 (98.15%)had surgery and the recurrence rate after simultaneously correcting nipple insulation was almost zero. Another 6 cases (5.56%)were also combined with nipple overflow, which reduced symptoms after treatment for surgical treatment. Conclusions This study suggests that 700 patients with plasma cell mastitis have high recurrence rate of direct surgery, anti-tuberculosis anti-inflammatory treatment is ineffective, control nipple overflow re-surgery, nipple depression correction recurrence rate is low, the efficacy is more accurate.
