目的 探讨滑膜组织中基质金属蛋白酶13(MMP-13)与β-catenin在骨关节炎(osteoarthritis,OA)严重程度中的评估价值。方法 选取解放军联勤保障部队第九二二医院2017-04至2019-04收治的OA患者92例,根据疾病严重程度将其分成轻度组(n=47)、中/重度组(n=45)。利用免疫组化法测定所有患者滑膜组织中MMP-13、β-catenin的阳性、阴性表达情况,并采用逆转录检测法测定MMP-13、β-catenin在滑膜组织中的表达量。绘制受试者工作特征曲线(ROC)分析MMP-13、β-catenin表达量对OA严重程度的预测价值。结果 轻度组的MMP-13、β-catenin阳性率分别为68.09%、55.32%,显著低于中/重度组的88.89%、75.56%(P<0.05)。轻度组的MMP-13、β-catenin阴性率分别为31.91%、44.68%,显著高于中/重度组的11.11%、24.44%(P<0.05)。轻度组MMP-13、β-catenin的表达量显著低于中/重度组(P<0.05)。MMP-13、β-catenin表达量预测OA严重度的曲线下面积分别为0.656、0.683。以MMP-13>0.887、β-catenin>1.975时,病情发展为中/重度的风险越高。结论 随着OA患者病情越严重,MMP-13与β-catenin阳性率越高,且二者在滑膜组织中的表达量越高,MMP-13与β-catenin表达量对OA严重度具有一定预测价值。
Abstract
Objective To investigate the value of matrix metalloproteinase-13 (MMP-13) and beta-catenin in evaluating the severity of osteoarthritis (OA).Methods 92 OA patients were selected from the 922 Hospital of PLA joint logistics support force from April 2017 to April 2019,According to the severity of the disease, they were divided into mild group (n=47) and moderate / severe group (n=45).The positive and negative expressions of MMP-13 and beta-catenin in synovial tissues of all patients were detected by immunohistochemistry.The expression of MMP-13 and beta-catenin in synovial tissue was determined by reverse transcription assay. The ROC was drawn to analyze the predictive value of MMP-13 and beta-catenin expression on the severity of OA.Results The positive rates of MMP-13 and beta-catenin in mild group were 68.09% and 55.32% respectively, which were significantly lower than those in moderate/severe group (88.89% and 75.56%, respectively) (P<0.05).The negative rates of MMP-13 and beta-catenin in mild group were 31.91% and 44.68% respectively, which were significantly higher than those in moderate/severe group (11.11% and 24.44%, respectively) (P<0.05).The expression of MMP-13 and beta-catenin in mild group was significantly lower than that in moderate/severe group (P<0.05).The area under the curve of MMP-13 and beta-catenin expression predicting OA severity was 0.656 and 0.683, respectively. When MMP-13 > 0.887 and beta-catenin >1.975, the higher the risk of developing to moderate or severe.Conclusions With the severity of OA, the higher the positive rate of MMP-13 and beta-catenin, and the higher the expression of MMP-13 and beta-catenin in synovial tissue, the expression of MMP-13 and beta-catenin has a certain predictive value for the severity of OA.
