高原驻训官兵肝功能损害状况及其与海拔高度的相关性

摘要

目的 了解高原驻训官兵肝功能状况,分析其与驻训海拔高度的相关性。方法 从四川阿坝藏区各驻训部队官兵体检报告中随机抽取575例军人的肝功能报告作为研究对象,根据海拔高度分为三组:<2000 m组144例,2000~3000 m组192例和>3000 m组239例。研究的肝功能指标包括总胆红素、间接胆红素、直接胆红素、总蛋白、白蛋白、球蛋白、白球比、碱性磷酸酶、谷丙转氨酶、谷草转氨酶、γ-谷氨酰转肽酶和总胆汁酸等。任何一项指标超出正常值范围即判断为肝功能异常。结果 575例官兵中,肝功能异常者198例(34.4%)。其中血清胆红素异常140例(24.3%),血清蛋白异常32例(5.6%),血清转氨酶异常70例(12.2%),总胆汁酸升高15例(2.6%)。分组研究显示海拔高度>3000 m组总胆红素、间接胆红素、总胆汁酸、谷丙转氨酶、γ-谷氨酰转肽酶、碱性磷酸酶、总蛋白、白蛋白和白球比异常的发生率显著高于<2000 m组和2000~3000 m组(P<0.05),而<2000 m组和2000~3000 m组之间上述指标异常的发生率无统计学差异(P>0.05)。直接胆红素、谷草转氨酶和球蛋白异常的发生率在不同海拔高度组之间的差异无统计学意义(P>0.05)。结论 高原驻训官兵肝功能异常较为常见,其中以血清间接胆红素、总胆红素、谷丙转氨酶异常为主。肝功能损害与海拔高度相关,主要发生于海拔3000 m以上区域,需重点关注。

Abstract

Objective To understand the status of liver function of officers and soldiers stationed on plateau and analyze its correlation with altitude. Methods Five hundred and seventy-five liver function reports were randomly selected from the physical examination reports of officers and soldiers stationed in Aba prefecture, Sichuan Province. According to the altitude, they were divided into three groups: 144 cases in the < 2000 m group, 192 cases in the 2000-3000 m group and 239 cases in the >3000 m group. The liver function indicators studied included total bilirubin, indirect bilirubin, direct bilirubin, total protein, albumin, globulin, white globulin ratio, alkaline phosphatase, alanine aminotransferase, aspartate aminotransferase, γ-Glutamyl transpeptidase and total bile acids. If any liver function indicator exceeded its normal range, it was judged as abnormal liver function. Results Of all the study objects, 198 cases (34.4%) had abnormal liver function, and the numbers of abnormal serum bilirubin, abnormal serum protein, abnormal serum transaminase and elevated total bile acid were 140 cases (24.3%) , 32 cases (5.6%) , 70 cases (12.2%) , and 15 cases (2.6%) respectively. The grouping study showed that the incidences of abnormalities of total bilirubin, indirect bilirubin, total bile acids, alanine aminotransferase, γ-Glutamyltranspeptidase, alkaline phosphatase, total protein, albumin and leukocyte ratio in >3000 m group were significantly higher than that in< 2000 m group and 2000-3000 m group (P<0.05), however, there was no significant difference between < 2000 m group and 2000-3000 m group (P>0.05). There was no significant difference in the incidence of abnormalities of direct bilirubin, aspartate aminotransferase and globulin among the three groups (P>0.05). Conclusions Abnormal liver function is common in officers and soldiers stationed at high altitude, and mainly manifested as the abnormalities of indirect bilirubin, total bilirubin and alanine aminotransferase. The damage of liver function is positively correlated with altitude, and mainly occurs in plateau area above 3000 meters, which needs to be paid special attention to.
