目的 探讨西帕依固龈液联合富血小板纤维蛋白膜(PRF)治疗慢性牙周病伴三壁骨缺损的效果。方法 选取2021-06至2023-01就诊于新疆军区总医院口腔门诊,确诊为重度牙周病伴三壁骨缺损者为研究对象,共计98例。按随机数字表法分为研究组与对照组(n=49),对照组采用引导组织再生膜(GTR)与人工骨粉植入进行治疗,研究组在对照组的基础上加入PRF膜与西帕依固龈液治疗,6个月后复诊,比较两组牙周探诊深度(PD)、附着丧失(CAL)、龈沟出血指数(SBI)、牙龈菌斑指数(PLI)、血清内炎性因子[肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、白细胞介素-10(IL-10)]水平的变化及治疗有效率。结果 与对照组比较,研究组PD、CAL、SBI、PLI水平降低,IL-10水平增高,IL-6和TNF-α水平降低;研究组总有效率(95.92%)显著高于对照组(79.60%),差异均有统计学意义(P＜0.05)。结论 西帕依固龈液联合PRF治疗慢性牙周病伴三壁骨缺损的效果显著,有效改善血清炎症因子水平及牙周状况。
Abstract
Objective To study the effect of Xipayi solid gingiva solution combined with platelet-rich fibrin (PRF) in the treatment of chronic periodontal disease with three-wall bone defect. Methods A total of 98 patients diagnosed with severe periodontal disease and three-wall bone defects were selected from the Stomatology Department of General Hospital of Xinjiang Military Region from June 2021 to January 2023. All the the patients were randomly divided into a study group (n=49) and a control group (n=49) according to random number table method. The control group was treated with guide tissue regeneration (GTR) and artificial bone meal implantation, while the study group was treated with PRF membrane and Sipayi gingival solidifying fluid on the basis of the control group. The changes of periodontal exploration (PD), loss of attachment (CAL), gingival sulcus bleeding index (SBI), gingival plaque index (PLI), the level of intragingival inflammatory factors(TNF-α), interleukin-6 (IL-6), interleukin-10 (IL-10) and the treatment effectiveness were compared between the two groups. Results Compared with the control group, the study group patients had lower levels of PD, CAL, PLI, SBI, IL-6, and TNF-α and increased levels of IL-10. The total effective rate of the study group (95.92%) was significantly higher than that of the control group (79.60%), with statistical significance (P<0.05). Conclusions Xipayi solid gingival fluid combined with PRF can effectively improve the level of serum inflammatory factors and periodontal status in the treatment of chronic periodontal disease accompanied by three-wall bone defect.
