目的 探讨全身免疫炎症指数(SII)与血清白蛋白(ALB)比值对急性胆源性胰腺炎(ABP)严重程度的判断价值。方法 回顾性分析武警安徽总队医院普外科2022-01至2024-12收治的136例ABP患者,依据修订版亚特兰大标准分为非重症组98例,重症组38例。检测患者入院24h内血常规及生化指标,计算SII/ALB、中性粒细胞与淋巴细胞比值(NLR)、血小板与淋巴细胞比值(PLR),同时对患者进行急性胰腺炎严重程度床边指数(BISAP)评分。通过ROC曲线比较SII/ALB、NLR、PLR及BISAP对ABP病情严重程度的判断效能。结果 重症组SII/ALB、NLR、PLR及BISAP评分显著高于非重症组(P<0.05)。ROC曲线显示,SII/ALB(AUC=0.804, 95%CI=0.727~0.882)与BISAP(AUC=0.806,95%CI=0.731~0.881)判断效能相近,均优于NLR(AUC=0.775, 95%CI=0.693~0.856)、PLR(AUC=0.643,95%CI=0.540~0.746)的判断效能。结论 SII/ALB可作为早期判断ABP病情严重程度的有效指标,具有较高临床应用价值。
Abstract
Objective To explore the value of the ratio of systemic immune-inflammation index (SII) to serum albumin (ALB) in judging the severity of acute biliary pancreatitis (ABP). Methods A retrospective analysis was conducted on 136 patients with ABP admitted to the General Surgery Department of Anhui Provincial Corps Hospital from January 2022 to December 2024. The patients were divided into non-severe (n=98) and severe (n=38) groups according to the revised Atlanta criteria. Blood samples collected within 24 hours of admission were analyzed for routine and biochemical parameters. The SII/ALB ratio, neutrophil-to-lymphocyte ratio (NLR), platelet-to-lymphocyte ratio (PLR), and bedside index for severity in acute pancreatitis (BISAP) score were calculated. Receiver operating characteristic (ROC) curves were used to compare the diagnostic performance of these indicators. Results The severe group exhibited significantly higher SII/ALB, NLR, PLR, and BISAP scores than the non-severe group (P<0.05). The ROC curve demonstrated that SII/ALB (AUC=0.804, 95% CI=0.727-0.882) and BISAP (AUC=0.806, 95% CI=0.731-0.881) had similar efficacy in judging the severity of ABP, and were superior to NLR (AUC=0.775, 95% CI=0.693-0.856) and PLR (AUC=0.643, 95% CI=0.540-0.746). Conclusions The SII/ALB ratio can be used as an effective indicator for early judgment of the severity of ABP and has high clinical application value.
